Claude Code v2.1.193: command classifier, path autocomplete, and memory management

The new autoMode.classifyAllShell setting routes all Bash and PowerShell commands through the auto-mode classifier. Rejection reasons now surface in the transcript, in a popup notification, and inside /permissions.

A claude_code.assistant_response event has been added to OpenTelemetry logging. It is off by default and enabled with OTEL_LOG_ASSISTANT_RESPONSES=1. Deployments that already log prompts will start capturing responses after the update, so set OTEL_LOG_ASSISTANT_RESPONSES=0 explicitly to preserve the previous behavior.

The release also adds live path autocomplete in bash mode and automatic memory cleanup for idle background shell commands. A ghost sub-agent appearing during background execution has been fixed.
