Claude Code 2.1.200: Manual mode is now default and background agent fixes

This update sets the default permission mode to Manual across all interfaces: CLI, VS Code, JetBrains, and the --help output. The previous flags --permission-mode manual and "defaultMode": "manual" remain supported. AskUserQuestion prompts no longer auto-proceed, and users can set a timeout via /config.

The bulk of the changes address background agents. The update fixes silent session termination after sleep, the accidental resumption of turns canceled via Esc, and a bug where agents failed to start due to stale daemon.lock files with reused PIDs. Daemon priority is now determined by build timestamps, preventing older versions from hijacking control.

Additionally, the release resolves a startup crash that occurred when disabledMcpServers or enabledMcpServers in .claude.json were not formatted as arrays.
